For your Livelihood™ registration today, you'll need to have the following information ready:

1. The legal employer's personal address, contact information, and Social Insurance Number (SIN). This could be your information, or the person you're applying on behalf of.

2. The employee(s)/caregiver(s) full name, address, birthdate, contact information and Social Insurance Number (SIN).

3. A payment card to process your Livelihood™ registration fee.

Once you have this information handy, fill out the form to the best of your ability. If you come across a question you don't have the answer to, feel free to insert N/A or leave the question blank.
